From my research https://spaffnerds.com/shows/2015-09-04 has it while https://spaffnerds.com/shows/2015-11-06 does not. It looks like this is the time frame when the band decided to drop the "Get ready to unite" verse.
Unfortunately we don't have audio for the handful of shows in between so we may never know exactly...

I am fairly sure it happened in AZ, and if I had to guess it was 11-6-15.

@RadiantDarkness this was also a special weekend because @Night-Train finally talked Brian into letting him hear the mythical Salt Lake jam. he hasn't been the same since.
